Our pathways

We commission delivery across structured pathways. Each is quality-assured, safeguarding-compliant and outcomes-measured through the Hub's central infrastructure. Commissioners buy pathways, not brands.

Live

Structured early intervention

Outcomes-measured, referral-based provision targeting emotional regulation, school avoidance, SEND-related needs, anxiety and low mood. SWEMWBS-tracked and commissioner-ready. For children, young people and adults.

In development

Therapeutic & specialist services

Qualified, accredited therapeutic provision at a higher threshold — clinical or semi-clinical. In development.

Live

Physical wellbeing & activity

Physical activity as prevention — movement, fitness and nutrition-based support for mental health. Targets sedentary lifestyles, confidence, and life transitions including menopause.

In development

Community wellbeing & connection

Lower-threshold, open-access activity tackling loneliness, isolation and social disconnection — for adults, retired people and carers. In development.

Live

Nature & environment-based support

Outdoor, land-based and animal-facilitated provision, aligned with green social prescribing.

Our Process

We manage every step from initial referral to final reporting. By matching specific needs with the perfect delivery partner, the Hub ensures clinical standards and quality outcomes are maintained for all funded and EHCP routes. See our current grant-funded cohorts on Funded Places →
